Case Study D - Frank's Profile Frank is a 79 year old male who lives in an aged care facility. He has always been able to complete tasks himself and was living independently in his own home. Last year, Frank suffered a fall and was not able to get back up again. His neighbours called the ambulance who came and took him to hospital. There he had an X-ray and it determined that he had broken his leg. His leg was put in a cast, and he had to use a wheelchair until his leg was healed. Since he broke his leg, Frank lost a lot of his independence and required support for the majority of his daily tasks. He moved to the local aged care facility and has been very happy with his decision. His children and grandchildren visit him weekly, and he feels very mentally stable. After the accident, Frank went to see his GP as he felt very weak. His GP sent him for a DEXA scan, also known as a bone density scan. The result of his scan was a T score of -2.8 which meant he was diagnosed with osteoporosis. Frank also has chronic kidney.
