Case Scenario: Matt is a 35-year male who was diagnosed with Crohn's disease 2 years ago. He was in the hospital for 2 weeks at that time. His initial work-up indicated acute disease in the last 5-7 cm of the jejunum and first 5 cm of the ileum. He has not had any flare-ups in his disease due to a drug regimen that included prednisone and sulfasalazine (anti-inflammatory drug). However, he now describes that over the past 2 weeks he has had more diarrhea, cramps and abdominal pain with the pain now unbearable. He has been admitted to the medicine unit for an acute exacerbation. CT imaging results revealed a stricture in his distal ileum. He went to surgery where an ileostomy was placed with resection of the stricture. Hopefully he will only have the ileostomy for 6-9 months at which time they will hook his ileum back up to his colon. A nutrition consult has been ordered for nutrition education regarding diet with an ileostomy. MIV is running at 120 mL/hour.
