Patient is a 26-year-old male presenting to the clinic for a follow up. Diagnosed of bipolar disorder. Assessment was completed and patient denies anger problem, irritability, mood swing, and inability to focus. Patient reports her was having problem of focusing the last time he visited and having problems at work. Patient reports he still has his job due to his stability. Patient states he smokes cigarette and does not drink alcohol due to the taste. Refill of medications and appointment schedule for 30 days. Geodon 80mg QHS and Depakote 500mg BID.
