You are seeing your 18 year patient with ADHD. She was diagnosed in early childhood after neuropsych testing. You have been working together for several years. She had a positive response to long-acting stimulants (Adderall XR) for many years and developed behavioral skills to also help her manage symptoms. She stopped taking medications in her last 2 years of high school because she felt like she no longer needed them. She made an appointment with you after her first semester at the local university. She would like to discuss restarting the medications. She is struggling to maintain attention and concentration. She feels so scattered and overwhelmed with the amount of work she needs to complete. She is becoming avoidant of the work because it takes all her energy to focus. She's not sleeping very well because her mind is worried about all the work she must do. She is afraid she will not maintain her grades and her parents will be disappointed and "disown" her. She feels left behind because her classmates seem to sail through their classes. "They all have A's. It's so easy for them." One of them mentioned that they take Adderall to get through their classes, so she thought she may need to restart her medication.
