Question

S James Smith is a 28-year-old male MRN 0001122 who has been admitted to the Intensive Care Unit with Diabetic Ketoacidosis and Acute Kidney Injury. Blood Glucose levels on admission to the emergency department were 600. B James Smith has a past medical history of DKA, Acute Kidney Injury, HTN and diabetes type 1. James' significant other reports patient has not been feeling well at home over the last 3 days, has had nausea and vomiting, unable to drink fluids and eat food, and has had a decrease in urine output. James significant other reports that patients' blood sugars at home have been 275-350 and has not been taking prescribed insulin due to not feeling well. A The patient has dry mucous membrane, decreased skin turgor and sunken eyes. Current vital signs are HR 130, RR 34, BP 130/78, T: 36.8, O2 sats: 96% on RA. The patient has Kussmaul's breathing. Urinary output is less than 0.5 ml/kg/hr. R Notify MD of urine output less than 0.5 ml/kg/hr. Start IV to begin fluid resuscitation, consider starting insulin drip to correct hyperglycemia.
