Patient Scenario:
Maria is an 83 y.o Hispanic female, admitted to the hospital with a diabetic foot ulcer to her right foot. Maria only speaks Spanish. Her glucose has been under poor control. She has had a diagnosis of peripheral neuropathy to both lower legs. She has had a longstanding PMH of Type II Diabetes, renal insufficiency, and osteoporosis. She has cataracts to both eyes, with an eye surgery planned for the next month. The patient has been living alone, with her children checking in on her periodically, and taking her to the doctor's office. Her eldest daughter Carmen has voiced concern that she has also has been "forgetting things" more often, and has seemed depressed the last month. The family is unaware of her recently having any falls, but they are concerned she "might break a hip" if she should fall. The patient can state her name, date of birth, but believes she is at her sister's house (the sister has been deceased for 5 years). The patient has an IV to her left wrist, with an antibiotic currently infusing. When the patient attempted to stand from the wheelchair to get into the bed, she was unable to bear any weight on her right foot. The family indicates that when the patient is discharged, she will be going to live with Carmen, as they feel their mother should no longer live alone. Just as you are leaving the room, the patient says she must use the restroom, but strongly resists the suggestion of using a bedpan.
