Question: Collect information from a consumer to support the development of a wellness plan. This will involve:
Providing information and resources that will enable the person to make informed choices about wellness planning
Developing a shared understanding of the person's goals, preferences and values
Working collaboratively to identify any cultural factors that may influence wellness planning
Working collaboratively to identify any person(s) who may need to be consulted regarding the wellness plan
Identifying, accessing and reviewing other information or documents that are needed for reference.
Support the development of a wellness plan. This will involve:
Developing a shared and whole of life understanding for what wellness means for the person
Collaboratively identifying previously useful approaches, factors and situations that promote and support
wellness for the person
Working collaboratively to decide the type of wellness plan to be completed and the items and areas to be included
Working collaboratively to identify factors, trigger situations and early signs of unwellness
Identifying options, potential strategies and the actions the person will take
Identifying other people to be included in the wellness plan and the actions they will take
Seeking agreement and consent from others according to legal and ethical requirements
Documenting the agreed plan
Support the development of an advance directive. This will involve:
Providing information and resources about advance directives, their purpose and legal standing
Collaboratively identifying the scope of the advance directive to be developed
Working with the person to decide what service, support, treatment and medication instructions or restrictions are to be included Collaboratively developing and documenting specific instructions
Jointly determining what life management or other instructions are to be included
Reviewing plan to ensure directive and instructions are clear and unambiguous
Determining the preferred way to confirm capacity at the time of Signing
Accessing legal and other specialist advice according to scope and nature of directive requirements
Working collaboratively to identify and locate a suitable willing independent witness
Identifying with whom and where the directive and any copies will be stored.
Identify a situation that requires a debrief, reflective practice or other support and initiate within scope of own job role.
