Assignment -

I - Improve overall communication between clincians and patients through MyChart usage between 5% by October 14, 2018 with the utilization of the new electronic SBAR application or tool.

The ultimate and sole purpose of this goal is to improve initial communication between clinicians and patients 24 hours a days 7 and days a week by having available access to a standardized electrnic SBAR application that allows both the clinician and patient to have open eneded communication with one another for any medical concerns, medication concerns, annd overall health concerns. The application will provide a set line for communication between clinicians and patients in which allows the clinclicans to have direct access to the patient's condition. Theapplication would be essentially very user friendly and always keeping the patient's best interest s in mind. The application would break down vital patient information while requiring healthcare professionals to give immediate responses within a timely manner this in return ensures that patient is being heard and not being overlooked.

II - Developing a more up to date SBAR application to the current MyChart application that is being used when clicians are comminicating with patients and other healthcare providers,

In 2006, the Joint Commission adopted an apporach to standardize the way communication is delivered amongst healthcare providers and patients and how it should be implemented. In response, many health organizations implemented many differented systems and methods to standardize communication errors or incomplete lines of communication associated with inaacurate information. Having MyChart allows the patient to recieve information on his/her health information day or nght in ample timing.
