For each diagnosis there should be primary prevention needs, secondary prevention needs tertiary prevention needs, and a S.M.A.R.T objectives for each diagnosis. The last column would be titled: Based on the diagnoses what are the resources needed to care for this client in the community located in Brooklyn New York zip code 11201.
