Assignment:

As a real or simulated workplace activity as directed by the assessor, you must assist three people with disabilities to community participation and social inclusion. This must be in accordance with their individualised plans.

You must work according to organisational guidelines, policies, protocols and procedures, to provide person-centred care. This must be within the boundaries of your role and under any supervisory arrangements that are made, including discussing, reporting and documenting work.

You must:

  • Work with each person to discuss and identify their strengths, interests, abilities and support requirements (this may be with the person's family or carer)
  • Research and identify appropriate opportunities for community participation and social inclusion for each person (as applicable to their needs)
  • Consider each person's needs, including their cultural and individual differences when matching services and opportunities
  • Work with each client (and any relevant others) to facilitate engagement with the appropriate services and opportunities, including providing information, discussing needs, identifying supports and barriers to participation and inclusion
  • Develop strategies to enable each person to participate and to minimise isolation
  • Identify and access resources, as available at the organisation and to each person
  • Support each person to develop community engagement in their individualised plan, with the necessary supports, aids and devices to maximise their participation
  • Assist each person to access community options, and opportunities with other community members
  • Support other workers to implement the individualised plans, as necessary
  • Identify and manage barriers to communication, as may be caused by cognitive deficits
  • Discuss travel and transport requirements and identify appropriate strategies to overcome any difficulties that may exist
  • Ensure processes are in place to evaluate and manage the ongoing success of each person's strategies for participation
  • Conduct location or activity risk assessment for community and social opportunities, as relevant to each person's needs
  • Determine any risk elements involved in accessing and participating in community and social opportunities, and discuss these with each person (and their family or carer, as applicable) to identify strategies that remove or reduce the risks to a safe level.
