Question

Joseph is in his late 80s and lives in an aged care facility in a semi-rural area. He lost his wife of 50 years six months ago, and he has been grieving for her ever since. His mood in the past few weeks has been low. He is quiet and resistive to care. He avoids talking much about himself and has withdrawn from the friendships and activities that he once enjoyed. Joseph is struggling with his own self-care, and an Aged Care Assessment Team (ACAT) assessment provided him with two hours per week of home help support. Develop and utilise case management processes in accordance with organisational policies and procedures.
