Health and Society Assessment Project

This assessment will provide students with the opportunity to develop an understanding of the social determinants of health and how these influence health outcomes across society. Propose an evidence-based intervention to address health disparities.

Task: Choose one Primary Health Network region of NSW and one current health issue within that region from the list below:

  • Diabetes
  • Obesity
  • Harmful use of alcohol

Explain the following:

  • The significance of the chosen health issue (e.g. how big a problem it is; the impact of the health issue on the people living within the Primary Health Network region)
  • The relationship between the health issue and at least one of the Australian National Health Priorities.
  • The relationship between the chosen health issue and three of the Social Determinants of Health

Use a SWOT (strengths, weaknesses, opportunities, threats) framework to propose a nurse led intervention that would promote improvements and/or good health for people within this region impacted by the chosen issue. The 'upstreaming' benefits of the intervention across diverse community groups should be included in the discussion. All work must be evidence- based and supported with references.
