Case

A man with an intellectual disability lives alone with his widowed mother. She has fallen and broken her hip and will be at the rehabilitation hospital for about six weeks. He cannot stay alone. He has a job at Goodwill Industries. County transportation takes him there every morning at 8:30 and brings him home at 5:00 p.m.

Develop a tentative plan for the client(s) and include answers to the following questions:

  1. Who is your client?
  2. What are his/her/their primary problems or immediate needs?
  3. What are three services you believe your client will need initially? Why do you think these services would be helpful or necessary?
  4. What are three services your client might use later once the case is stabilized? Why do you think these future services might be helpful or necessary?
