Introduction

Nurses working in mental health are required to undertake thorough and systematic mental state and risk assessments in a variety of settings with people in various age groups. There are many and varied assessment tools used in the mental health setting; however, a sound working knowledge and understanding of the Mental State Examination (MSE) and Risk Assessment are essential. These assessments will be completed after your participation in a role play that is developed, written as a transcript, and acted. The role play will involve members of the Multidisciplinary Team (MDT) and the client from your role play.

This assessment aims to determine your knowledge and understanding of the MDT meetings in the mental health setting, the MSE and Risk Assessment for people with a mental illness.

The sections in the MSE provide an overview of the person's present mental state and will be used throughout the care process. The Risk Assessment considers the psychological (e.g. current mental health) and social factors (e.g. relationship problems, employment status) as part of a comprehensive review of the person to capture their care needs and assess their risk of harm to themselves and or others.

Summary
Students will work in groups of four (4) to develop a script and role play from one (1) of the provided scenarios. Your scenario must be different to other members of the group. The role play will be delivered and assessed in the classroom. The script for your role play and the accompanying documents for the MSE and risk assessment must be uploaded to Blackboard. For this assessment, you must complete the following:

1. Develop a role play where you are the Enrolled Nurse participating in a MDT meeting. The members of the MDT are listed in the Introduction and the Assessment Brief.
2. Complete the Mental State Examination according to the script that you have developed for the consumer.
3. Complete the Risk Assessment according to the script that you have developed for the consumer.

• Each student is to choose one (1) scenario; every student in the group must have a different scenario. You will work with a group to deliver your role play. The members of your group will play the roles listed in the brief.
• Read your scenario carefully as there are several questions that need to be incorporated into the role- play script.
• The role play is one (1) assessment for the unit Implement and Monitor the Care of a Client with a Mental Health Condition; thus, each student must be the Enrolled Nurse in their own script with a transcript and role play that meets the criteria to be deemed as satisfactory for this assessment.

• Application of nursing theory to mental health context and values and philosophies of mental health nursing.
• Support the rights and dignity of the person with a mental health condition.
• Recognise the biopsychosocial effects in the person with a mental health condition.
• Consumer and carer perspectives on mental health care are noted and considered.
• Recognise the impacts that racism, discrimination, negative stereotyping and stigma can have on a person with a mental health condition and ensure interactions with the person are positive.
• Challenging behaviours in the meeting are managed as per hospital protocol and are evidence- based.
• Mental health legislation (jurisdictional) and consumer rights.
• Comprehensive care, wellbeing of the young person and adolescent, and nursing interventions have been planned, prioritised and implemented with the contribution of the person.
• Medication relevant to the mental health condition.
• Resources and education materials that are related to the specific mental health condition, what effects this can have to physical health and are relevant to the state you are in. (QLD/NSW/VIC/SA).
• Recovery principles are applied with the support of the MDT for optimum outcomes.
• Key mental health terms used.


Scenario 1
You are an EN working in the Adolescent Mental Health ward. This morning, you are caring for Nia who will be discharged in one week. You have been asked to attend the Multidisciplinary Team Meeting with the following people:
• Nia.
• Nurse Practitioner, Eating Disorders Team (Registered Mental Health Nurse).
• Dietician.
• Enrolled Nurse (must be played by you).
Character Briefs
Nia was diagnosed with Anorexia Nervosa 12 months ago and has been managed successfully in the community for the past 10 months. However, increasing weight loss and anxiety resulted in her voluntary admission about four weeks ago.
Nia has responded well to treatment and attended school in hospital. Nia has gained weight and is medically ready for discharge; however, in the last few days, you have noted changes in her mental health that concern you. Nia has no history or thoughts of self-harm. Nia has no family history of mental health conditions.
In your transcript, you will need to include:
• Signs and symptoms of depression and anxiety.
• Verbal and non-verbal communication from Nia that covers the areas in the Mental State Examination and the Risk Assessment.
• Nia will respond negatively at times in the meeting with behaviours that indicate distress.

The Enrolled Nurse (played by you) will discuss and demonstrate the following:
• Nia's progress in terms of patient engagement in the Daily Activities Program.
• Adhering to the ward rules, strengths, personal contribution to the ward activities and willingness to attend the dental department during her stay.
• Discuss 2 medications (use generic names) that Nia has commenced since her admission listing side effects (2 side effects for each) and how they have been managed.
• Health promotion strategies (relaxation techniques, exercise, rest, sleep etc.) that Nia can use in recovery.
• Support programs in the community including local services and national helplines.
• Ensure that Nia is supported; informed of her rights; understands that her medication regime, treatment and care are relevant for a young person; and is comprehensive in terms of patient options and overall physical and mental wellbeing.

The Nurse Practitioner (NP) for the Eating Disorders Team will coordinate the meeting. They will also manage the escalating behaviour of Nia in the meeting demonstrating de-escalation strategies and techniques for managing behaviour as scripted.

The Dietician is a health professional who will complete the nutrition assessment and conduct ongoing monitoring. The dietician will discuss diet, lifestyle and balance between study and rest, and an outpatient follow-up appointment with the nutrition team.

Ensure you have used mental health terms in the script.
