Want to develop a portal where a user logs in using their credentials. so in db we will need an area to create login for the users.

We will needs their login id, Full name, Email address, Centre, and password. Once they login to the portal they get 3 categories of forms as attached. (this is just the link). Once they click on appropriate link they get the form.

The form will carry forward the centre name on the top and at the bottom as footer will have the user that is logged on and the current data and time. Form (here i again where db is required) (DATA) - Centre - this is pulled out automatically from the User logged in because we have already stored the centre name on his login. any data that we can prepopulate from login table can be prepopulated in these forms.

Date and time to be as current system time but can be changed manually. we can put tht image as static on the form as tht wont change. thts the format for report too. Cancel Submit buttons This would send the form as submitted but not approved.

The user needs to go to the Approval Pending section, click on tick box and submit to mark that a approved.

Also, need buttons for Print to print the form in a particular format and Extract to extract to word and Excel. Lastly, will need additional reports to extract data - Reports - incidents in centre (we should be able to specify start and end date and also centre names, incl All centres).

Also, approved reports for the same and approval pending report.

Can you confirm a few elements: a) Reporting is working and can get reports centre wise. b) a user can submit as many incidents as they want and not 1 incident per user. c) After a user submits the incident or mediccation or illness, it goes into pending state and the same user can go in the Status section and click on checkbox to make it Approved and Completed (from Pending State). d) A admin user should be able to see all reports and get reports for all or each centre or all or each user or month etc. e) A logo and the images on the form I sent are on the pages. f) On the Incident Form, there are two images, one for body and other for head, users can click on the area of the body that had injury or have checkbox which says head, shoulder, arm, legs, etc. Pls confirm these because there are the main elements of the system.
