Mental Health Assessment:
Aim of assessment
The aim of this assessment is to give students feedback on how well they demonstrate an understanding of the topics and applications of Promoting Mental Health and Wellbeing AND you will also be required to develop a plan of care to address the identified problem.
Details
This assessment will provide an opportunity for you to demonstrate your understanding and ability to communicate the key practice issues involved in providing quality and evidence based care of individuals with a serious mental illness. This is a closed book, in-class examination that will be undertaken in your week 5 tutorial class under exam conditions as per university policy.
The practice issues that will be examined include:
1. Mental status examination (MSE)
2. Nursing care planning
3. The therapeutic use of self and therapeutic communication - critique of the interview
Procedure
You will be shown a 15-20-minute video trigger twice during the tutorial time. Notes may be taken while watching the video trigger. Video has English subtitles.
Activities to be completed and submitted through Turnitin by Midnight within 2 days of viewing.
Activity 1
You are required to carry out a Mental Status Examination (MSE) of the client interviewed in the video trigger, using the following headings:
Appearance
Behaviour
Mood
Affect
Speech
Thought form
Thought content
Perception
Cognition: memory (short-term and long-term), concentration and orientation to time, place & person.
Insight
Judgement
Risks: You need to identify 4 risks related to the client interviewed in the video trigger
Activity 2
You are also required to complete a nursing care plan for the video client addressing the highest priority problem/need
(one problem) as identified by you as follows:
- Need/Priority problem
- Explain why the nominated need/problem is of a high priority to your client
- Goal (what you would hope to achieve as a result of your interventions)?
- Three (3) nursing interventions/strategies to support the client in meeting his/her goal, with a rationale for
each intervention
Activity 3
You are required to provide a comprehensive written feedback (a critique) of the nurse's interaction with the client, identifying four (4) positive and two (2) negative elements of this interaction. Please provide feedback on the process of the interactions between the nurse and the client (example: The nurse used silence appropriately while waiting for the client to respond). You need to cite examples from the video to support this.
Do not comment on the content of the interactions (example: The nurse did not ask any question about the client's mood or thought content). This is already covered in Activity 1. Communication skills and care planning will be covered in the tutorials prior to this assessment.
Activity 4
Meet academic writing and referencing standards within text and with reference list .
Important details about your assessment
- Refer to marking criteria and standards for mark allocation for each question.
- This assignment does not require a standard essay introduction or conclusion.
- There is a word limit of 2000 words. Use your computer to total the number of words used in your assignment.
However, do not include the reference list at the end of your assignment in the word count. In-text citations will be included in the additional 10% word count. If you exceed the word limit by more than 10% the marker will stop marking at 2000 words plus 10%.
- This assessment will be marked on-line and therefore no hard copy is required.
- You are required to organise your answers in order of questions by referring to the question number in your answer.
- Marks will be allocated for academic writing and referencing (see marking guide)

- Student is to submit according to the submission instructions in this guide.

- If misadventure prevents you from attempting the assessment within the allocated period, please follow the Western Sydney University procedures for Special Consideration
- Submissions received 30 minutes past midnight on the due date will incur late penalty
- If you do not attempt the assessment, you will receive zero (0) and will fail the unit for non-submission of an assessment task

APPEARANCE ( physical description, hygiene, grooming, hair colour and length, posture, poise, body type, looking stated, notable characteristics such as tattoos, piercings)
BEHAVIOUR( rapport, engagement, psychomotor activity both qualatitive and quantative, attitude )
AFFECT (observed emotional response - appropriate, restricted, flattened, congruence)
MOOD (reported feeling or emotion as expressed by the client, including depth, intensity , duration and fluctuations)
SPEECH (quantity, rate, volume, tone, flow,unusual characteristics)
THOUGHT FORM (the process, how they put ideas together, logical, tangential, concrete, blocked, vague, poverty, loose associations)
THOUGHT CONTENT ( what the person is actually thinking, detail themes, obsessions, delusions, ideas, plans, antisocial urges )
PERCEPTION (hallucinations - auditory, visual, somatic, gustatory, kinaesthetic, olfactory - identify type describing client experiences )
COGNITION & INTELLECTUAL FUNCTIONING ( orientation - time, place and person; memory - remote, recent, recall ; attention , concentration; ability to plan and implement; fund of knowledge; calculations; abstract reasoning )
INSIGHT &JUDGEMENT ( insight = awareness of problem - denial; judgement = understanding of consequences of their behaviours )

RATIONALE FOR CHOSEN ISSUE ( outline the impact of this issue on the clients wellbeing if this issue is not addressed, what information from the case study identified this as an issue, use literature AND client information to support your choice)
GOAL/S(what will your ultimate outcome be for the above problem, direct relationship to the issue identified )
INTERVENTIONS - 3 ONLY (the interventions MUST be nursing interventions, be client specific, short term ,detailed -what, how, when /frequency,SUPPORT validity with literature)
RATIONALES -( the how / why of interventions , for each of the interventions use literature that supports the validity of this intervention and its relationship to being able to achieve your goals and what the outcome on the client should / could be )

What did the interviewer say or do, why and how was this good or bad with regards to communication and a therapeutic relationship and give the reason why it is good or bad - use literature to support ) 4 positive interactions and 2 negative interactions .
