Problem

You are to develop a comprehensive case management plan for a 19-year-old autistic male whose primary caretaker- mother- recently died. He graduated from high school, has a high GPA, and skills in art and design. His social skills, communication, and daily life routines need assistance. Be certain to include the following components (it will work best for each of these to be level two headings-see APA manual)

I. The person's life stage and needs with clear rationale.

II. Use the I AM tool to specify how to assess the individual's preferences, hopes and dreams.

III. Identify supportive routines to be considered- including daily life and employment/ volunteer activities.

IV. Describe where, with whom and how the person lives-- housing and living options, community access, transportation, home adaptation and modification.
