Presentation - The student will develop a literature based community profile linked to a client/client group of their choice. This evidence-based assessment will require the student to choose a client, and identify the range of community-based support available.

Report - A commentary on challenges and responses to the delivery of care to diverse population groups.

You chose a relevant topic of your assignment. You include an introduction which does indicate how the work will develop.

The next section of your work struggles to meet the course criteria and is primarily a discussion of peer support and a recognition of the perceived stigma of a diagnosis of dementia.

You do briefly mention a community profile in the later section of the work, however, this is not referenced. You also suggest few places to support people with dementia.

You briefly discuss preventative services. Your understanding of these is not clear. You do not discuss a range of services for those living with dementia. Confusion between UK and Welsh legislation/policy.

You do not include a client profile. Wales on line is not a strong reference.

In the last section you do highlight some partnership work within the Cardiff Council area. You include a summary and a conclusion. This was not necessary. You have some minor referencing errors.
