Develop a comprehensive nursing care plan for a 67-year-old patient admitted with acute decompensated heart failure. The patient has a history of hypertension, type 2 diabetes, and chronic systolic heart failure. On admission, the patient complains of worsening shortness of breath, orthopnea, paroxysmal nocturnal dyspnea, and peripheral edema. Vital signs show blood pressure 145/92 mmHg, heart rate 110 bpm, respiratory rate 28 breaths/min, and oxygen saturation 92% on room air. Physical exam reveals crackles on lung auscultation, S3 heart sound, 2+ pitting edema of the lower extremities bilaterally. Laboratory results show elevated BNP, low hemoglobin and hyponatremia. Chest x-ray shows pulmonary vascular congestion.
