1. Develop a comprehensive client vignette based on the following requirements:
a. A vignette. Your vignette should include a clear clinical problem experienced by a client you will be providing treatment interventions. Your vignette should have the following characteristics

i. Client has clear name, gender, age, place of birth, sexual orientation, relationship status and job, occupation, or academic status (family, intimate, significant friendships, etc.)

ii. A DSM 5 Clinical disorder or Personal problem that you clearly & fully describe being careful to create sufficient details that clear signs and symptoms of the problem are evident in the vignette as well as how the disorder or problem is impacting the client's functioning. Be sure the symptoms match the requirements of the DSM 5 diagnostic criteria

2. Using the SOAP note format taught, develop a SOAP note based on the vignette you develop and provide.

3. Conduct a functional assessment of the client's presenting problems and provide an analysis of what you believe is going on with the client. Provide a written rationale for why you believe the patient should be given a specific diagnosis being sure to cite all the evidence and examples from the vignette that support your beliefs about the diagnosis you have identified for the client / patient in the vignette.

4. Based on your analysis of the symptoms and appropriate diagnosis, develop a comprehensive treatment plan that clearly identifies at least 3 separate treatment goals along with their associated objectives and interventions for each of the three goals developed. Be sure that your objectives are measurable and include time frames for completion. Describe what you would do to adjust the treatment plan if the client is not meeting a specific goal's objective. Also be sure what professional staff position (for instance the social worker, the psychologist, etc.) will be responsible for helping the client achieve each of the three goals.

Be sure to present each treatment Goal with its own objective and interventions. For instance,
a. Treatment Goal 1
b. Objective 1
c. Intervention 1
And so on.....

Also, be sure the objectives and interventions used help support the Treatment Goal they are designed to address.

5. Provide an additional brief statement that clearly summarizes how you will determine if your goals/ objectives are working for the client/patient, Be specific.
