Assignment:

The purpose of this clinical assignment is to provide nursing students with an opportunity to apply their knowledge and skills in ensuring safe transitions of care for patients across various healthcare settings. This assignment will focus on effective communication, medication reconciliation, and patient education to promote continuity of care.

This assignment can be completed as a slide presentation, a paper, or a concept map. Your instructor will assign this as either an individual or small group project.

Part 1: Preparing for Transition

Select a patient from the clinical setting who is scheduled for a transition of care (e.g., hospital to home, hospital to skilled nursing facility, etc.). Ensure that the patient has complex healthcare needs that require careful coordination during the transition.

Conduct a thorough review of the patient's medical history, including diagnoses, current medications, allergies, and any special considerations for the transition.

Develop a comprehensive care plan for the patient's transition. Include the following components:

• A detailed summary of the patient's current health status and specific care needs during the transition.
• A medication reconciliation plan, including any adjustments or changes needed for the transition.
• Identification of potential barriers or challenges in the transition process and strategies to address them.
• A communication plan that outlines how information will be shared among healthcare providers involved in the transition.

Part 2: Implementation of Safe Transitions

Discuss how you would address the following issues related to transition of care for your patient.

• Coordinating with the healthcare team (physicians, pharmacists, therapists, etc.) to ensure a smooth transition for the patient.
• Communicate any concerns or special considerations identified during the preparation phase.
• Conducting a medication reconciliation with the patient, ensuring that they understand their medications, dosage instructions, and any changes made during the transition. Document any discrepancies or issues that arise during this process.
• Engaging in effective communication with the patient, their family, and the receiving healthcare facility. Ensure that all relevant information is shared accurately and in a timely manner.
• Providing education to the patient and their family about their post-transition care plan, including any self-care tasks, follow-up appointments, and warning signs that require immediate attention.
