In an effort to reduce damaged good and improve productivity, Fabricast Incorporated started a workforce study. After surveying groups of employees to understand the causes of low quality, the results were not conclusive (employees were reluctant to give true answers and were not clear in many cases).
Fabricast began the study by thinking in theoretical terms and came up with a number of conditions they can use to correct the behaviour. A committee then narrowed the ideas to a short list of four conditions which probabilistically would work with their people, that place and at that time. The study started in month 8. The target was set at 50 damages (per 1,000).
In month 8, program one was started. In month 13, program two was started. In month 15, program three was started. In month 18, program four was started.
