Social determinants of health are non-medical factors that can affect an individual's health. These include economic stability, education access and quality, health care access and quality, environment, and social or community context. Social determinants of health can lead to health disparities that prevent individuals from having access to quality health care.

Studies have determined that social determinants of health can increase the risk of exposure, degree of susceptibility, and the course and outcome of an acute or chronic illness or diseases (Cockerham et al., 2017). For example, smoking or lack of exercise often initiates the onset of pathological processes that lead to chronic diseases such as COPD, diabetes, or heart disease. Those who face social determinants of health are more likely to engage in risky behaviors that result in disease.

The communicable disease chain model represents the fundamental idea that illness or disease transmission occurs when all six links are present. The six links of disease transmission are pathogen, reservoir, portal of exit, mode of transmission, portal of entry, and susceptible host (Green, 2018). If these six links remain present, transmission of illness or disease will continue to spread from host to host. Interruption in this chain will assist in preventing the spread of illness or disease. For example, a nurse who performs hand hygiene between every interaction with patients prevents the spread of infectious agents by breaking the communicable disease chain model because their hands can act as a mode of transmission.
