Question: Design an individualized DM Multidisciplinary Team and a specific management plan for a DM patient. Must come up with either a theoretical patient or may find an actual DM patient to serve as basis for developing the management plan. Include the following: Patient profile and brief nursing history Members of the team Health care professionals to be included in the team Number of professionals you will need (e.g. how many nurses, doctors, etc will be needed) Professional qualifications of each member Specific roles and responsibilities of each member Organizational chart or chain of command of each member (include where the patient is to be placed) Goals and objectives of the team General schedule of activities to be done for patient management (e.g. when should the patient meet the professionals, scheduled check-ups or meetings). You can schedule this on a monthly basis for a period of 1 year.
