1. State the overarching program goal;
2. State the long-range health objective;
3. List the intermediate behavioral objective(s);
4. List the Process objectives (stated in terms of program activities to be implemented by staff)
a. Provide a detailed description of each major program activity outlined by these process objectives.
i. The description of each activity should provide information on "how much of what will be done to whom by when and where."
ii. The description should discuss the intervention's strategies, implementers, and setting(s), including the rationale for selecting them based on a review of intervention research studies b. As part of that description, state the anticipated Outputs (not in SMART format, counts of things that demonstrate implementation).
5. List the immediate program objectives (these should be stated in terms of participant expected change as a result of the abovementioned program activities)
