Case Study: Arati is a 79- year-old woman who, lives in a self-managed residential aged-care facility and whose health has declined over the last 15 years with a history of poorly controlled type 2 diabetes for which she takes metformin 500mg BD.

Arati has been admitted to the high dependence unit after a severe allergic reaction to a medication she had taken.

After taking a physical assessment you find out that Arati is overweight, history of DVTs, does not pay much attention to her blood sugar levels or her diet and leads a relatively sedentary lifestyle and mentioned that lately she notices dizziness or light headedness towards the end of the day.

You overhear the nurses and doctor discussing that Arati and has also developed elevated serum creatine levels indicating renal impairment.

After a visit by the medical practitioner (GP) it is noted that Arati's blood pressure is elevated and she has protein and glucose in her urine. This leads to further investigations where she is diagnosed with chronic renal failure Grade 4.

The medical practitioner suggests that a new assessment be conducted to determine Arati's level of care requirements within the residential aged care facility.
