Problem: R.N is a 45-year-old woman who is married, has three children under the age of 12, a dog, works full time and part time, is a student, and has a cleaning business on the side. She was at work and started experiencing severe nausea/vomiting along with severe stomach pains. She went to the emergency department where they did an x-ray of her abdomen and administered some nausea medication. The x-ray resulted in severe constipation and she was admitted to the acute care medical/surgical floor with orders of NS at 125/hr, stool softener PO meds, a suppository, an enema, laxatives, a high fiber diet, and to push fluids. Answer the following questions. Why do you think this happened to R.N? What is an appropriate nursing diagnosis for this scenario (nursing diagnosis R/T AEB)? Out of the bowel care options above, what order/orders would you initiate first? How would you educate R.N in regards to future prevention? Describe the proper order to perform abdominal assessment along with how long you listen to bowel sounds for. If there are no bowel sounds, what could be happening?
