Claire Label is 89 years old. she was diagnosis with Chronic Obstructive Pulmonary Disease, unspecified (primary diagnosis) and was admitted to the nursing home. during her stay she was diagnosed (Secondary diagnosis) with weakness, chronic atrial fibrillation, unspecified, types 2 diabetes mellitus without complications, chronic kidney disease, stage 3 unspecified, essential (primary) hypertension, and dependence on supplemental oxygen (she is on oxygen 2l nasal cannula). Claire Label B.M had a shortness of breath, she is at risk for falls due to deconditioning, bowel and bladder incontinence, periods of confusion, use of cardiac, narcotic, diuretic, and anticonvulsant medication.
