He has had headaches for the past several years but they are becoming more frequent, increasing to approximately 2-3 times a month. The headaches are precipitated by vision changes and slight alteration in smell. He describes the pain as sharp and mostly in the frontal region. He rates his pain as 6/10 on the pain scale. The pain does not radiate. The headache started yesterday but continued through the night despite taking 800mg Ibuprofen. He reports some nausea but no vomiting. Vital signs: T 98.9, Pulse 112 beats/min, Respirations 18 breaths/minute, and BP 126/84 mmHg.
