Choose a client and health care promotion concern.
The client may be a family member or a friend. Identify this client by age and initials only in the paper. Identify language, cultural background, and developmental age, but withhold full identification of the client. Then, identify the health promotion concern of the client.
Select an assessment technique or tool, in addition to the nursing process, to gather data about the identified health promotion concern. Briefly describe the assessment tool. (reference this tool in the reference list, and cite).
Collect the data from the assessment and describe the findings. Identify the abnormal findings or concerns of the client.
Collaborate with the client and interpret the data. Describe the key points in the process of collaboration. Prioritize the data and collaborate with the client to recommend the desired outcome. Document findings.
Recommend interventions to meet the desired outcome. For example, if the client wants to start a walking plan three times a week, helping the client determine what the best approach would be a type of intervention.
