I. Description of Intervention (Program/Service) Selected

i. Select a program or service (Intervention) that you are going to evaluate in your community or from your field placement (agency).

ii. Briefly describe the intervention, program, or service that you plan to evaluate and identify the core components.

II. Problem Identification & Purpose of Evaluation

i. Identify and describe the problem or need your evaluation aims to address. See page 113 table 6.1 in your textbook which highlights the difference between a problem and a need.

ii. Conduct a critical review of the empirical literature on the problem or need that your selected program/intervention addresses; and literature on programs, interventions, or services that address the problem or need (if available). Include Three empirical sources, at least. Justify why it is important to address the problem or need using evaluation. Please use data and research findings to support your justification. Describe the diversity of your population and its relevance to why you choose the group.

iii. State the purpose or aim of the evaluation.

III. Types of Evaluation Chosen

i. Select type(s) of evaluation that you are going to conduct. It could be formative or summative or both. Note: This may not apply to needs assessment.

ii. State why you chose the evaluation type(s) and describe characteristics, including core components, of the evaluation.

IV. Target Population

i. Identify the group or groups affected by the problem or need and/or who will benefit from the intervention, program, or service being evaluated.

V. Goals & Objectives of the Program/Intervention

i. Describe the goals and measurable objectives of the program.

ii. Identify any specific activities/strategies for achieving the goals and objectives of the program.

iii. Using the logic model, explain the theory or assumptions guiding your intervention, program, or service to achieve its goals & objectives (e.g., change, prevent, or treat the specified problem).

VI. Evaluation Research Design

i. Specify the research design planned for the evaluation and why it was selected. I.e., Single System Designs, Group Designs, Pre & Post

Test designs, etc.

ii. Describe how you conduct your evaluation by applying the research design chosen.

iii. Describe how the research design will address the goals and objectives, and outcomes.

VII. Sampling

i. Describe the type of sampling that will be selected and why it was selected.

ii. Identify the inclusion/exclusion criteria used for selecting participants and a rationale for the criteria.

iii. Describe how you will recruit and retain participants.

iv. Briefly explain how you will protect the rights of participants (e.g., ethical issues of informed consent, voluntary participation, and protection of sensitive data).

VIII. Data Collection

i. Describe how and where you will get your data for the evaluation.

ii. Specify whether new, primary data or secondary data.

IX. Conclusion

a. Identify the strengths and limitations of the planned evaluation. Please give examples to support your response.
