Question - You are living and working as a Community Health Nurse in a remote village in Mali.  The village is located along a small branch of the Niger River.  There is no electricity or running water in the village.  Families live in small thatched huts.  Cooking is done outside over a fire.  The Niger River provides abundant water and is used for drinking water, washing and dumping of refuse.  There are no latrines.

You have been asked by the lay health worker to accompany her to the home of a 6 month old infant.  The infant lives with her 18 year old mother, who also has 3 other children.  The infant's father has left to try and find work in the nearest large town, which is a 2 day walk from the village.  The mother reports the infant has been ill for 3 days with diarrhea.  The rest of the family also became ill after a dinner of poorly cooked meat.  The other family members have recovered.  The mother reports she has not breast fed the child in 24 hours for fear of making the diarrhea worse.
