Mr. Bernard is a 90-year-old resident of a long term care facility. He tells the nurse he has an ache in his right foot. He suggests he might have stepped on something or twisted his ankle. "Maybe I got bit by a bug when I was outside yesterday". The nurse notes the medial aspect of Mr. Bernard's right ankle is reddened, slightly swollen and warm. His temperature is within normal limits. He has a strong pedal pulse bilaterally. Bernard's ankle is x rayed and there is no fracture noted. He has full range of motion in his ankle and lower extremity, although the pain in his ankle increases with movement. A Doppler ultrasound rules out deep vein thrombosis.
