Case SCENARIO

You are the community nurse visiting Jimmy Tien to review his medications this week as some of his medications had been changed. Jimmy is a 75 year old man with a history of chronic obstructive pulmonary disease, transient ischemic attacks, hypertension and mild osteoarthritis in his hands and knees. His wife died of breast cancer 2 years ago. Jimmy lives with his daughter Nguyen Qui, her husband and their children. Jimmy is sociable, likes to go out and grows most of his own vegetables.When you see Jimmy you notice he has a moist cough and looks exhausted. You ask him how he is feeling and he says 'not too bad'. You go into the kitchen where Jimmy sits down. His medicines are in disarray when they are usually organised.Jimmy's daughter Nguyen Qui arrives during your visit to have lunch with her dad. She asks him why he is still in his pyjamas. He is usually showered and dressed when she visits. You notice his breakfast is uneaten on his bench top. You take a set of vital signs which show a deterioration since the last set. You make the decision to transfer Jimmy to the local hospital ED following your assessment. He is diagnosed with pneumonia.
