Patient: Hispanic 34y/o with severe dyspnea that started 2 weeks ago and progressively worsening. BP 110/60, HR 120, Temp 101, RR 40, O2 88% on room air, temp of 101. O2 at 4L/NC improved to 92%. Symptoms include: fever, chills, weight loss, malaise/fatigue, diaphoresis, cough, dyspnea, weakness, swelling in lower extremities, feeling of abdominal fullness. Social hx includes: Bisexual, IV drug use in past, and heavy drinking in the past. Denies current smoking/drug use.
Physical assessment shows white patches on tongue and buccal mucosa bilaterally, dyspnea, lungs have rales in all fields, hepatosplenomegaly with shifting dullness, wide spread lymphadenopathy, with a line of vesicles on left thorax that extends from anterior to posterior thorax.
