Samantha described herself feeling "lightheaded" and "anxious" on the drive to the clinic. Samantha's school and peer experiences were recently linked to anxiety and depression (Walden University, 2022). Her anxiety has been through the sky since entering high school, but she's found respite via long jogs. Samantha described yet another occasion in which she collapsed at school. Because her heart was racing so fast, she had difficulty breathing throughout her first high school exam. Samantha's insomnia was thought to be caused by her anxiety over being vaccinated.
Design a care plan for Samanthat Polaski case that includes:
- Two identified problems
- Three or four goals
- Three or four interventions
- Justification for the care plan (i.e., Why is it appropriate for the patient?)
- Explanation of how you would advocate for this plan within the IDT and achieve buy-in from the patient, caregiver, and other members
