The case study for this outcome relates to hypertension. Josephine Green has gone to her family physician as she has been experiencing headaches and is not feeling "quite right'. You are the home care nurse and will be assessing her blood pressure and providing teaching.
Scenario
Josephine Green, 75-years-old, is a recent widow. She came to Canada from Uganda as a small child. She has two children and 6 grandchildren living in the same city. Her health history includes arthritis and bilateral cataract surgery.
She and her husband used to go for daily walks but, since his death 2 months ago, she has little desire to go outside her home. She has been experiencing headaches and not been feeling "quite right" the last couple of weeks. She went to her family physician for a physical examination. This is where it was discovered that her blood pressure was elevated - 180/95. The physician has ordered Home Care Services to assess her blood pressure every day for one week.
You are the home care nurse and are assigned to visit Sharom to monitor her blood pressure and provide teaching. You are greeted by Josephine at the door of her house. After introducing yourself and explaining the purpose of your visit, you perform a set of vital signs on Josephine. Her BP is 175/98, HR 89, Temp 36.5, Wt. 77 kg, and Ht. 157 cm. Josephine is a little anxious and wants to know about hypertension.
