Case: The goal of my proposal program is to provide community resources to clients once they are discharge from Gateway Community Services. gateway provides services for individuals struggling with substance abuse and mental health. Two outcomes I want to achieve is all staff trained in completing discharge treatment plans that include assisting with community resources and decreasing the number of readmissions. My goal is to reduce the readmissions to atleast thirty percent within the first two years, properly train all staff in discharge planning and community resources provided. These outcomes will support proving resources for clients once discharged by making sure clients feel prepared once they are discharged and are aware of the community resources provided.
