Part A: All About Me
My Autobiographical Cover Sheet--Front
Your cover should convey something about you. You may use drawings, pictures, photos, or symbols that relate to your personality or interests. Your first and last name should be prominently displayed.
My Autobiographical Cover Sheet--Back
Write a brief explanation (minimum of 3 sentences) of your cover's relevance (complete sentences, coherent paragraphs, etc.).
*Both parts of the cover will be graded for effort and neatness. Points will be deducted for any information that's missing, including the heading material.
Part B: My (your) Choice
Choose any THREE of the suggested topics listed below and write a paragraph reflection on each subject.
The title should be from the prompt you selected. For example,"The Questioning Me," "The Relaxing Me," etc.
Consider any or all of the questions/prompts seen below. You may answer more about the topic than those questions listed under it, but must answer all the questions listed beneath it at minimum. Minimum requirements: Typed, ONE paragraph in length, double-spaced about THREE of the topics on pg. 2. Spelling and grammar will count for points.

Topic Choices for "My Choice"
Animals and Me: Write about an important animal(s) in your life. Explain why is this animal special. Share interesting stories about the animal.
Regret(s) and Me: What regrets do you have? Consider both past actions and "the roads not taken." If you could have a do-over for anything you have said or done in your life, what would you do differently? Why?
Myself and _______: Can you think of a category not listed here? If you'd like to write an element of you not listed, ask me for approval first.
The Questioning Me: Consider a question about your life that is important to you. What are your dreams? Hopes? Worries? Explore a "big" question.
My accomplishments: Describe a challenging project you completed at some point along with an explanation of how you became interested in it (e.g. : sewing, writing a story, building a birdhouse, etc.) If possible, include a photograph, blueprint, or drawing to explain the project.
My Goals: Describe a goal or series of goals. Why is it your goal? What drives you to reach it? How do you plan to reach it? What steps do you need to take?
The future of Me: What are your short-term and long-term plans for a job or career? What leisure time activities do you anticipate in the future? Where and how do you plan to live?
My Family and Me: Describe your family members and what you like about each one. Include pictures if you wish. What has each member of the family taught you?
The Relaxing Me: What do you do to relax? How do you eliminate or reduce stress? To unwind? Discuss your favorite forms of entertainment.
Humor and Me: Describe your sense of humor. Give (appropriate) examples of cartoons, jokes, TV shows, movies, etc. that have made you laugh. Share stories where your sense of humor came into play and/or a practical joke story.
Me Back When: Share mini-stories of yourself in the past. Choose an incident that you feel changed who you are or impacted your life. What, specifically, changed you?
Nature and Me: Describe how you relate to the natural world and what you like seeing or doing outside. Why is this activity important to you?
Sports and Me: Describe your love with sports or a sport. Do you enjoy playing it and/or watching it? Why? Write about an intense moment during a sport you played/witnessed. Which team are you most devoted to?
Why? What's the best game you've ever seen?
Art, Music, or Movies and Me: Choose either art, movies, or music specifically and tell me who you like about that topic (artist, actor, singer, etc.). What inspires about you? How are you affected when you paint/listen to music/play trombone/watch Johnny Depp/etc.?
