a. PHYSICAL: Refer to information on the checklist to determine what to watch for.  

(include: age, general health, eating habits and nutrition, any physical concerns or injuries, immunizations, general description of physical characteristics of the child, fine and gross motor skills, games with rules, rough-and-tumble paly, dominance hierarchies, sleeping habits, )

b. Describe child’s immediate setting (inside/outside, space, lighting, furnishing, toys available, neighborhood, community…).

c. SOCIAL/EMOTIONAL: Use the Developmental Checklist to guide your observation: Is the child alone or with others?  Describe the child’s interactions with the adults in the room.  With the other children. What type of relationship does the child seem to have with others? (examples of Erikson stage, temperament, attachment, emotional self-regulation, emotional expressions, self and gender recognition, empathy, interactions with adults/peers, moral choices, self-esteem, friendships, play interactions, attitude toward successes and failures, confidence, emotional problem-solving skills,  …)

d. COGNITIVE DEVELOPMENT: List the children’s activities that took place while you observed one child.  Include activities of any adults in the area.  (Piagetian stage, imaginary and fantasy play, language development and vocabulary, inquire/ exploration/curiosity, logical thought, spatial reasoning, memory strategies, reading level, understanding of number concepts,  written language and symbols, problem solving, planning, cognitive self-regulation…)

e. RUNNING RECORD: Observe the child for five to ten minutes.  Record everything the child said and did during that time.  Use quotation marks for direct quotes.  Is the  child interacting with anyone/anything?  Does the child appear energetic/listless?  (Use other side if necessary)

 5. Evaluation: What conclusions have you drawn from your observation about the child’s development? What are your areas that you are considering addressing through an intervention?  (brainstorm and list several with any additional thoughts)
