Design either a survey (quantitative - using a Likert Scale type instrument which has investigators ask questions in a way that has participants rate responses in terms of how much the agree or disagree with a statement), or a questionnaire (qualitative - which asks open-ended questions beginning with WHAT, HOW, or WHY gaining narrative answers to gain information from participants) to be used to determine job satisfaction within a worksite. Be sure not to ask questions that might suggest a certain positive or negative response, and be sure to design your instrument so as not to create fear in answering the questions. Explain at the top of your instruments that participants are free to answer, no, some, or all questions without consequence, and that all information shared is kept confidential.
