Assignment:

Tom is a resident in a disability care facility. He was recently placed in care by his wife Ethel, because he was involved in a motor vehicle accident which left him with physical and mental disabilities. This has affected his behaviour. He was once a gentle, quiet man, but he now reacts to various situations with physical and/or verbal aggression. Tom used to be a chef and he was an avid gardener. Tom can clean his teeth and dress himself but he needs support with other personal care needs. He is continent, and he is able to manage simple tasks but he has not yet been assigned to any activities or group sessions.

Section 1.

  1. How might staff support Tom to develop and follow daily routines? Provide six examples.
  2. One morning Tom shows resistance to engaging in the cooking activities that he usually enjoys. He has been asked to help to make jelly.
  3. Identify possible problems with engaging him and identify people staff could seek appropriate assistance from.
  4. Briefly describe an environment that would be conducive to Tom's positive and adaptive responses, identifying five of its characteristics.
  5. Provide three examples of reinforcements that might be used to encourage Tom to make his bed.
  6. Describe how staff could contribute to the development of a behaviour support plan to manage Tom's aggressive behaviour, which could put Tom or others at risk of harm.
  7. What sort of review should be carried out and documented if it is felt that the environment is having an effect on Tom's behaviour? Where should recommendations from the review be documented?
  8. The ABC chart has been compiled from reports from staff about Tom's behaviour on his first day in care. Describe four strategies that might be identified in Tom's individual response plan as a result of the information provided.

Section 2.

  1. What types of interventions would be inappropriate in Tom's case? Provide 10 examples.
  2. Tom becomes frustrated with you one morning because he is telling you some stories about his past and he feels that you are not listening to him. How could you demonstrate that you are listening to him in the future? List and describe eight examples.
  3. One afternoon Tom becomes very aggressive. He is angry because he thinks another client stole some of his belongings. He attempts to physically attack the person and when you step in, Tom lunges at you and says he will 'strangle you if you do not go away'. How should you respond to this incident in order to ensure the safety of yourself and others?
  4. Write an incident report for this event.

  • strategies to determine changes in Tom's needs and behaviours
  • the effectiveness in developing and maintaining positive and adaptive responses
  • the reduction of risk of harm to Tom and others
  • the level of intrusion on Tom's dignity and self-esteem
