Rewrite: Patient was seen via telehelth and she consented.The patient has communicated that her depression symptoms persist, and she believes the SSRIs are ineffective. She continues to experience low energy, lack of motivation, sadness, hopelessness, and irritability, even though she has been taking her medication as directed. She has requested a change in her medication. It's important to note that the patient has previously tried various SSRIs. She has not expressed any suicidal or homicidal thoughts and denies experiencing any psychotic symptoms. We have mutually agreed to initiate a treatment plan with Cymbalta.
