Case Study
One of the plants you are working at is experiencing some difficulties with a hose connection on a holding machine that keeps a metal part in positions while it is fed into an automatic embossing machine. The clamp does not always allow the metal part to be fed into the machine because of the lack of pressure in the hose, which aids with properly aligning the items being embossed. The worker who was operating the machine had 40 years of experience with this type of equipment.
One day while attempting to fix the alignment of the machine, without shutting off the power, the worker reached into the machine. He placed one hand between the alignment feeder mechanism while his other hand was positioning the misaligned part between the open clamps.
Unfortunately, the worker accidentally pushed the button that causes the machine to run the embossing process with his hip, while his hand was in the machine. The machine cycled, and the worker had his arm dislocated and was the recipient of a 4th degree burn from the middle of his hand to the halfway point of his forearm.
