Episodes of Care (EOC) are holistic assessments which enable you to demonstrate progression across a number of platforms. Effective communication and relationship management skills underpin all aspects of care.
In Part 1 examples of an EOC could be considered to be one occasion of assisting someone with their personal hygiene (in acute care); or changing a wound dressing (in community care) / giving a vaccination in a GP surgery.
In Part 2 examples of an EOC could be undertaking a health assessment with individuals with multiple comorbidities (community); or delivering care for a bay of patients in an acute ward or conversations surrounding mental capacity and Do Not Attempt Resuscitate decisions in a GP setting.
Two EOC must be achieved by the end of each part, however in Part 1 and Part2 one is formative and one is summative. In Part 3 both will be summative. By summative we mean that these will receive an academic grade.
In all Parts the EOC must be available to be discussed with your Practice Assessor as part of your final interview for each placement.
At your final interview your Practice Assessor will discuss your EOC with you (They will also discuss their observations of you delivering your episode of care) and assign a grade (eg fair, good etc), this will be different from the academic grade which will be a percentage score.
The patient I was looking after is a bariatric patient with very high BMI, suffering from diabetes, heart failure and her body was swollen due to heart failure. I made a referral for her to the dietitian and I had chat with regarding healthy eating which she is compliant at the moment. She is diabetic type 1 and she is on insulin; her sugar levels are high due to eating lots of sweets. I had asked for permission to take her sweets away and she agreed. I had to signpost to the other useful website eg eatwell guide NHS. Also, had a conversation with her husband in regards to healthy eating regime. I had to make a referral to the tissue viability nurses because she had sores all over her body. Discharge is complicated, physiotherapist and I went to see her house, the main issue was her passage was very tiny but we managed to find alternative way by using the back door. Also, adaptations were made, fitting the hoist in her house, and new bariatric bed was ordered, chair and commode. This patient is in rehabilitation and she not engaging well with exercises.
