Simulation Based Assessment Task:

A patient you were allocated to provide nursing care for is getting discharged this afternoon. The person requires GP and dietician appointments to plan further management of his blood pressure and diabetis mellitus diagnosed during the time of hospitalisation.

The person's next of kin has a disability and uses crutches to ambulate. The next of kin is anxious about the person's home care and seeks support from you.

Demonstrate how you will ensure the patient has all discharge planning requirements including a General Practitioner appointment, dietician referral and any medications in place.

Demonstrate how you will support the next of kin addressing their emotional and physical needs to better manage the person in care.

You may request a classmate to play the role of the patient. Your assessor will play the role of the next of kin. The time allocated for this assessment is twenty minutes.
