The survey should include at least six questions that:
has a relevant title
has a client's details section which provides space to record:
the client's name
what services they received
the date the survey was taken
can gauge the client satisfaction:
allows answers based on a five-point scale
allows clients to suggest possible improvements or other services.
Note: Your will need to evaluate the responses and create graphical representation in the next task.
Step 2
Using the template available via the CBSA website, create an email for the survey and send the email to five clients (you will need to send this to five of your classmates to complete) and copy Glenda Williams, include the link to the online survey with a due date for completion.

the survey - include a copy of your survey questions in this folder
the survey results
evaluations of survey results.
You must provide limited secure access to the storage system for yourself, Glenda Williams (your assessor), and nobody else.
