Case Study: Tim a 70-year-old who lives with his husband john. John is a full-time worker and travels for long periods. Tim is a recently retired real estate agent and has a history of chronic back pain, hypertension, and constipation. He is overweight with a BMI of 30.5. His blood pressure managed by two anti-hypertensives, and his chronic pain analgesics, have the side effect of constipation for which he takes multiple aperients. he states that he loves his post-retirement life, enjoying puzzles, Netflix, reading and walking his dog. He sometimes can travel with his partner when he is away for work. he has current referrals to a physiotherapist, dietician, occupational therapist and has appointments scheduled with these members of the allied health team within the next 6 weeks.

Questions:

  1. Concisely summarise the case study and define the chosen modifiable risk factor behaviour.
  2. Outline how the risk factor, if left unchanged, may impact on the disability outlined in the case study (i.e. pathophysiology), and how the further decline/loss of independence may impact the person's life, considering their personal circumstances.
  3. Identify a health behaviour theory that could be used as the basis for an intervention to modify that behaviour (such as, but not limited to: health belief model, theory of planned behaviour, transtheoretical model, health promotion model). Concisely outline the theory, and how you, as the RN, would encourage behaviour change using this theory.
  4. Outline and justify one (1) innovative and practical strategy that can be implemented at home by the person in the case study to encourage risk factor behaviour modification, why it is a relevant strategy for the case study, and what the RN's role will be within this strategy.
  5. Outline and justify one (1) appropriate (real) service/support/group in your area able to assist. Include a brief description of the service (relevant to the clinician, not the consumer), why this service will assist, and what the RN's role will be within this strategy.
