You have a patient that is 12 weeks gestation with a past medical history of epilepsy. She is currently taking an anti-epileptic drug. Interventions for this patient would include

Prescribing daily folic acid. Giving daily oral Vitamin D to decrease the risk of bleeding Decrease lab monitoring of the anti-seizure drug levels to reduce bleeding risk. Stopping the anti-epileptic medication to decrease the risk of congenital abnormalities.
