A leadership-driven organizational "culture of safety" is comprised of, but not limited to, the following essential elements:

Recognition that we "live in a high-risk environment" and that errors can happen to anyone at any time;

A commitment to the fact that Patient Safety is everyone's job - every minute, every day;

A "Just Culture" of transparent and open reporting of errors and near misses; and

Key leadership participation and support.

The "culture of safety" does not happen by accident

There are important, well-researched actions that leadership can take to create such a culture. Based on your readings and research, what do you think are the three most important actions that you, as a leader, could can take in your organization to create "a culture of safety"?
